Entry 01 — Where clinics get stuck

You run a clinic, not an ad account. Meta's medical review pulls campaigns offline for weeks, cheap leads never walk in, and the privacy changes since iOS 14 hide what's working. Most clinics we talk to are stuck between accounts that keep getting flagged and agencies that ignore compliance entirely.

Observed across every account we audit. The playbooks blur; the categories don't.

Entry 02 — What we know

A different operating system.

Meta isn't broken for med spas.

Consultation booking runs on its own mechanics: show-up rate, deposit capture, and a medical review that flags what most agencies miss. The patterns below decide whether a med spa campaign survives review and books. Our process is built around them.

Five patterns that break Meta campaigns in this category, and how we handle each.

Finding 01 / 05

Restricted creative gets you flagged.

How we handle it

Every asset is cleared against Meta's medical policies before it publishes. The result-photo formats Meta tends to pull are reworked into split-screen carousels that pass review, and primary text names the outcome without the restricted terms.

Finding 02 / 05

Cheap leads don't show up.

How we handle it

We measure cost per booked consultation, not cost per click. A deposit at booking raises the share of booked leads who walk in.

Apple's privacy changes hide what's working.

How we handle it

Conversions API alongside the pixel, plus CRM-level attribution that ties booked appointments back to the original ad. We report booked patients per dollar, not what Meta self-reports.

Finding 04 / 05

Lookalikes degrade on a poor seed.

How we handle it

We seed from high-value patient lists, CRM exports of patients above a spend threshold, not raw pixel events. A value-weighted seed outlasts an interest-only lookalike.

Finding 05 / 05

One creative burns out in a week.

How we handle it

Four to six variants per ad set, angles rotated weekly, with frequency caps. Testimonials, result photos that pass review, treatment education, and offer creative each rotate.

Entry 03 — Tested in cycles. Reported weekly. Earned monthly.

We run Meta Ads in disciplined cycles.

  1. Log 03.1Days 01–10 · Testing

Every campaign runs in cycles of seven to ten days. Each one closes with a written log of what we tested and what we learned, and the next cycle starts from it. The first thirty days are slow while we map the account. After that, the cycles compound.

  2. Log 03.2Friday · Reporting

Every week, a five-minute Loom: what we tested, what changed, and why. Watchable on your phone, no meeting required.

  3. Log 03.3Continuous · Guardrail

Meta's medical review hits every new creative, so we plan for it. Assets are pre-cleared, result photos run as carousels built to pass, and no protected health information goes through the pixel.

  4. Log 03.4Monthly · The stake

Targets are set together in month one. Miss them and the next month is on us, tracked in your booking system, not ours.

Entry 06 — The operator

The operator behind Haldwen
Plate 06 — The operatorOn every account

Haldwen is a Meta Ads practice for med spas and boutique Pilates studios. Single-operator by design. One person runs the testing cycles, records the weekly Loom, and takes the monthly call. No handoff from the person who closed you to a junior you never met. A small operation holds a few accounts at a time, and each one gets attention a thirty-person agency can't give it.

* The person who closes you runs your account. No handoff, no juniors.

Entry 07 — Index of questions

Questions, answered before the call.

We don't guess at compliance. We track what passes Meta's review in this category, what gets pulled, and what runs for months, and that informs every brief. A verified Business Manager and aged accounts are setup conditions on our side.

We start with a compliance and account-health audit. If your account is restricted, we map the path to reinstatement, or to a clean verified Business Manager, before spending a dollar. Running on a flagged account just burns budget.

We optimize to cost per booked consultation, not cost per lead. A deposit at booking, reminder flows, and CRM attribution show which ads produce patients who walk in, and we cut the ones that don't.

Conversions API alongside the pixel, plus CRM-level attribution that ties booked appointments back to the original ad. We report booked patients per dollar from your booking system, not what Meta self-reports.

Targets are set together in month one. If we miss them, the next month is on us, tracked in your booking system, not ours. You keep everything we produced.

Compliance setup and the first learning cycles take time. The first thirty days map your account, the next sixty are where the patterns compound. Three months is the shortest window where the work can prove itself.

We work best with clinics spending roughly $8,000+/month in ad budget, separate from our retainer. Below that there isn't enough volume for the cycles to compound.

We can run alongside an in-house team or replace an agency that's underperforming. What we won't do is split one account's strategy across two parties. The compliance and testing discipline only works when one operator owns it end to end.

Most major clinic platforms: Boulevard, Aesthetic Record, Mangomint, Vagaro, Acuity and similar, plus Meta's Conversions API. Reporting ties back to whichever system you already run on.

Onboarding runs about two weeks: a compliance and account-health audit, voice-and-offer mapping, and tracking setup. First campaigns go live once the account is verified and the pixel and Conversions API are confirmed.
